Shampoo for hair loss

Shampoo against hair loss is one of the most sought-after products in the care category, and at the same time one of the most misunderstood. For expectations to be realistic, it helps to understand what a shampoo can and cannot do by its very nature.

What a shampoo actually does

A shampoo is a product that is applied, emulsified and rinsed off. The contact time with the scalp is measured in tens of seconds, rarely more than a minute or two. Its basic function is cleansing, removing sebum, sweat, styling residue and impurities from the scalp and hair.

Everything else a shampoo offers is built on that basis. Conditioning components stay on the hair and change it mechanically: they reduce friction, make combing easier, close the cuticle. That has a measurable effect on how much hair breaks during combing and drying, which is not the same thing as growth.

Shedding and breakage are not the same thing

This is the key distinction that marketing routinely blurs. If a hair comes out of the follicle with the root, it is shedding, a process taking place in the scalp. If a hair snaps along its length, it is breakage, and the cause lies in the condition of the hair itself: chemical treatments, heat, mechanical strain.

Shampoo and conditioner can meaningfully affect breakage. Hair then looks thicker and keeps its length longer, simply because fewer strands snap. The ingredients most often mentioned

Caffeine

Caffeine is probably the most recognisable ingredient in this category. It has been studied in vitro and in smaller clinical papers, and the results are debated, partly because of sample sizes, partly because contact time in a rinse-off product is short. That is why caffeine is more often seen in leave-in formulations as well.

Ketoconazole and piroctone olamine

These ingredients target the fungus of the genus Malassezia and the condition of the scalp. In seborrhoeic dermatitis and dandruff, reducing inflammation and scaling improves the environment in which hair grows. Shampoos with ketoconazole in higher concentrations have the status of a medicine and are dispensed in pharmacies; cosmetic shampoos contain lower concentrations of other antimicrobial ingredients.

Saw palmetto, plant extracts, niacinamide

Some ingredients are added for a presumed effect on the scalp or for an antioxidant profile. The evidence is uneven and mostly comes from small studies. The presence of an ingredient in the composition says nothing about its concentration or about whether it is functional at all in a rinse-off formula.

Sulfates and silicones

Both have been the subject of marketing demonisation for years. Sulfates are surfactants: they wash. For very dry, coloured or damaged hair, milder surfactants may feel more pleasant, but a sulfate in itself does not cause hair loss. Silicones coat the hair and reduce friction; they do not suffocate the follicle, but in some hair types they build up, so an occasional deeper wash is useful.

How to read the label

Ingredients are listed in the INCI list in descending order of concentration down to the one per cent mark. If an ingredient highlighted on the front of the packaging is at the end of the list, after preservatives and fragrance, its proportion is very small. That does not necessarily mean it is useless, because some ingredients are active at low concentrations, but it does mean that the headline on the packaging should not be read as a promise.

It is also worth paying attention to the wording on the packaging. A cosmetic product may not claim to treat disease. If a package claims to “cure alopecia” or “bring back lost hair”, that is a sign that the text does not comply with the rules, not a sign that the product is stronger.

A realistic approach

A shampoo is the foundation of a routine, not its active part. What proves sensible in practice:

  • Regular washing adapted to the scalp type: an oily scalp is washed more often, and that is not harmful
  • Applying and massaging into the scalp, not along the lengths; the lengths are washed by the lather running down
  • Conditioner on the lengths and ends, not the roots, to reduce breakage
  • Gentle drying and limiting high temperatures
  • If there is dandruff, itching or redness, treating the scalp before anything else

Products that stay on the scalp, such as serums and ampoules, have a considerably longer contact time and are therefore used in a routine as the carrier of active ingredients, while the shampoo stays in the cleansing role.

When a shampoo is not the answer

If the hairline is receding, if the crown is thinning in a pattern or if hair is suddenly falling out in large amounts, the cause is almost certainly beyond the reach of a rinse-off cosmetic product.   • A shampoo primarily cleanses; its contact time with the scalp is short.
