Biotin for hair

Biotin appears so often in the marketing of hair products that it has almost become a synonym for “hair vitamin”. The picture from the scientific literature is considerably more modest than the one on the packaging, and it is worth knowing before you buy.

What biotin is

Biotin is vitamin B7, also known as vitamin H. It is water-soluble and acts as a cofactor for a group of enzymes involved in the metabolism of fatty acids, glucose and amino acids. Through amino acid metabolism it is also connected to the synthesis of keratin, the structural protein that hair and nails are made of, which is the source of its reputation.

Biotin deficiency is rare

This is the most important fact in the whole story. Biotin is found in a large number of foods: eggs, liver, nuts, seeds, legumes, salmon, dairy products, sweet potato. In addition, it is produced to some extent by the gut microflora.

In people who eat a varied diet, biotin deficiency is practically not encountered. The conditions in which it is described are most often:

  • Hereditary disorders of biotin metabolism, such as biotinidase deficiency
  • Long-term use of certain antiepileptic drugs
  • Long-term regular consumption of raw egg whites: avidin from raw egg white binds biotin and prevents its absorption; cooking breaks down avidin
  • Long-term parenteral nutrition without added vitamins
  • Chronic alcohol consumption and severe malabsorption

A true deficiency does not show only in the hair. It is accompanied by a scaly dermatitis around the eyes, nose and mouth, conjunctivitis, and in pronounced forms neurological symptoms as well. Isolated hair shedding in an otherwise healthy person is almost never the result of biotin deficiency.

What the research says

Review papers that looked at biotin supplementation for hair and nail problems reach a consistent conclusion: the evidence of benefit in people without a deficiency is weak. The cases of improvement described in the literature concern patients with a proven deficiency or with specific hereditary conditions.

In other words, biotin makes up for what is missing. If nothing is missing, an extra amount has nowhere to act: the surplus, since the vitamin is water-soluble, is excreted in the urine.

A practical problem: biotin interferes with laboratory tests

This is a detail rarely mentioned on the packaging, yet clinically significant. High doses of biotin, of the kind found in many hair supplements, can seriously distort the results of laboratory tests that use biotin-streptavidin technology.

The results most often affected are thyroid hormones and troponin, the marker used in assessing a heart attack. Falsely altered results can lead to a wrong conclusion in either direction. Regulatory bodies in several countries have issued warnings about this.

So: if you take biotin, always tell your doctor and the laboratory before a blood test. The usual practice is a pause of several days before testing, but the decision on that is made by a doctor.

When biotin makes sense

It makes sense in situations established by a doctor: with a proven deficiency, with hereditary disorders of biotin metabolism and with conditions that impair absorption. In those cases supplementation is meaningful and targeted.

What has no basis is taking high doses “just in case”, in the hope that more means better. With water-soluble vitamins that calculation does not work.

What to check instead with hair shedding

When hair thins, it is more useful to look at the parameters that in practice really do turn out disturbed more often:

  • Ferritin and iron stores, particularly in women
  • Thyroid function
  • Vitamin D
  • Total protein and energy intake, especially with restrictive diets
  • Zinc, in conditions with malabsorption

Biotin in cosmetics

Biotin is also found in the composition of shampoos, serums and other hair preparations. The situation there is different from supplementation: this is topical application, in small concentrations, with limited penetration through the skin. Its role in such formulations is above all conditioning rather than nutritional. A cosmetic product, including scalp serums, does not correct a vitamin deficiency in the body.

In short

  • Biotin is vitamin B7, needed for metabolism and keratin synthesis.
  • Deficiency is rare and as a rule has both skin and neurological signs, not only hair shedding.
  • In people without a deficiency the evidence of benefit from supplementation is weak.
  • High doses can seriously distort laboratory tests: tell your doctor.
  • With hair shedding it is worth checking iron, thyroid function and protein intake first.
  • Biotin in cosmetics acts topically and does not make up for a deficiency in the body.
